Helpful Forms
If you are a new client, please download, print and complete all the following intake forms and bring them with you to your first session.
Please arrive 10 minutes early to your appointment and have your printed and completed forms with you, as well as your ID and insurance card. I will need to take a copy of your ID and insurance card.
Also, please make sure that you have verified your coverage with your insurance provider and be prepared with your copayment to be paid by check or cash only.
There is a 24-hour cancellation policy.
Therefore, if you do not cancel with at least 24-hour notice prior to your appointment time, you will personally be responsible for the No Show or Late Cancellation Fee for that missed session. Please be sure to cancel or reschedule 24 hours or more in advance to avoid this charge.

Please note: There are some forms that are required for MINORS under the age of 18 in addition to the required ALL forms. Adults over the age of 18 will fill out the required ALL forms and those labeled ADULTS.
